Tennis instruction registration (EXAMPLE OF NEW WORKFLOW)

If student is also enrolled in Extended Learning After-School care, please be sure to sign and submit THIS WAIVER to Expanded Learning staff in order to participate in both programs.

The staff of Goleta Valley Tennis Academy is not CPR certified.

Pete - this is a different registration page. It is set up to record all registrations in a Google Sheet and then send a confirmation email to the person who registered/submitted the form.

Please test this as if you are the parent. You won’t get the normal confirmation email. The goal is to see if you like the confirmation email that you asked for. If you want something different in the confirmation email, let me know.

Terms & Policies

  • If class must be cancelled due to weather, holiday, or instructor's absence the class will be made-up the week following the last class on the regularly scheduled class day and time.

  • The instructor will notify students by phone if class is to be cancelled. If there is no notification students are to assume class will take place.

  • Students may make up a missed class by attending another class already in session provided there is space available in that class. The make-up class must take place during the same session period in which the student is already enrolled. Please check with the instructor for scheduling a make-up class.

  • Balance of total payment must be received four weeks prior to arrival in order to guarantee space reservation.

  • Students may sign up late for a class already in session at a pro-rated cost provided space availability.

  • Group lessons meet for four consecutive weeks.

  • Private & Semi-Private lessons are scheduled by appointment.
